Each service is designed to help you or your loved one live as independently as possible, with optimal quality of life, while avoiding facility transfers and hospitalizations. Home and community-based services (HCBS) provide opportunities for people enrolled in Medicaid to receive care in the home as directed by the agency. The federal government program provides services to the elderly, people with disabilities and clients with mental illness who need Medicaid in-home care in a familiar setting.
