Pathways Youth Transitional Living Program

Cornerstones of Care provides 30+ trauma-informed programs and services to children and families throughout Missouri who have experienced trauma that can affect their mental, physical and behavioral health with long-term and life-altering consequences. The Pathways Transitional Living program provides support, guidance, housing and case management services for young adults in foster care, involved in the juvenile justice system or who are experiencing homelessness. This program connects young adults to resources, services and opportunities that will enhance their future.
